- The paper attempted to examine the reasons accounting for the sustenance of the practice of female circumcision and how to stem it. The basis of the facts presented here are derived from a survey involving a sample of four hundred and five respondents of both sexes conducted in two local government areas (Okpe and Sapele) of Delta state. The subjects that constituted the sample were randomly chosen and the data analysed through simple descriptive and inferential statistics. The results showed that the persistence of female circumcision is due to the application of western methods in the operation thereby reducing the agony and stress associated with the practice. It also identified the low level of education and urbanisation as factors inducing the sustainability of the practice because they make for the persistence of traditional norms and values. Consequent upon these findings, the paper concluded by suggesting the involvement and enlistment of western trained health care professionals as public health educator in an attempt to stamp out the act. In addition to the above, the paper suggested the rapid development of our educational system and urbanisation as a way of weakening non functional traditional values. 2 results 2
- "This study examines how federal restructuring and decentralisation can be tailored towards the creation of an enabling environment for business in Nigeria. It does this by making a comparative assessment of the business environment in Nigetia; exploring the character of federalism and decentralisation programmes in Nigeria. It also examines the implications of these for governance and the business environment in Nigeria while suggesting decentralisation reforms required to enhance government efficiency and effectiveness as well as improving the business environment. The study noted that decentralization programmes in Nigeria began from the colonial era but have largely taken the form of spatial deconcentration. As a result, they have had the effect of increasing central control and reducing opportunities for citizen participation, and stultified creativity. The restraint in devolving power is attributable to the effort at regime sustenance in the face of limited state legitimacy, the problem of fragile national unity and the prevalence of military dictatorship with its centralizing tendencies. Territorial fragmentation and internal boundary adjustments have resulted in a proliferation of states and local governments; but such levels of government have been without local power that can attract and stimulate participation. Spatial deconcentration has resulted in a bloated states sector, with minimal private sector development, and a suppression of innovative and entrepreneurial energy. Dependence of sub-national units on oil revenue from the centre has been one of the major reasons fur the failure to diversify the economic base of the country. The competition that had characterised inter-state relations under the three and four-region Systems in which derivation was a significant factor of horizontal revenue sharing gave way to political struggles for federation funds. Thus, local spending became completely separated from local resources in the name of even development across the country. The centralisation of resources control and the adoption of a general revenue allocation formula provided no incentive for competitiveness among the various sub-national governments. Decentralisation reforms have become imperative but need to be done democratically. There should be wide spread consultation and negotiation to reach consensus on an ideological base for the envisaged decentralization programme. There should also be very clear institutional arrangement for managing the process, realistic and clear-cut distribution of powers and functions among the various governments based on the principle of subsidiarity with clearly spelt out institutions of horizontal and vertical accountability. The programme should be informed by a more practicalconcern about economic competitiveness, such that the country will be restructured into competing governmental units, providing room for public/private partnership in productive activities at the lower levels, such that would enable the exploration and development of economic potentials of the various states. The current general revenue sharing formula should be reconsidered with a view to promoting competition hand in hand with the drive for equity and accountability. Caution should be taken during the process to avoid fanning the embers of centrifugal forces. Nigeria should borrow ideas from successful decentralisation efforts in other lands. " 1 results 1

- Background/Aims: Literature suggests that aerobic exercise improves cognitive impairments post stroke. This systematic review was conducted to analyse evidence on the effectiveness of aerobic exercise in improving post-stroke cognitive impairments. Methods: Online databases (PubMed, EMBASE and Web of Science) were systematically searched from inception until 13 July 2017 using the keywords stroke/exercise/cognition. Clinical trials that met the inclusion criteria were assessed for methodological quality using the PEDro scale. Extracted data were synthesised for evidence. Results: A total of seven studies met the inclusion criteria. Participants in most of the studies were aged over 60 years and the majority had ischaemic stroke. The most commonly used measure for assessing cognition was the Mini Mental State Examination. The majority of studies included moderate to high intensity exercise (50–70% of VO2max) for 30–60 minutes three to five times per week. There is moderate evidence that aerobic exercise enhances global cognitive function, attention and working memory. Evidence that aerobic exercise improves memory, levels of brain-derived neurotrophic factor and executive function is conflicting and limited. Conclusions: Aerobic exercise is moderately effective in improving post-stroke cognitive impairments. More clinical trials are needed in view of the methodological limitations and paucity of existing studies. 1 results 1
- Background: Cognitive rehabilitation (CR) is an important component of stroke rehabilitation. Most of the strategies used for this purpose are not readily affordable. This is the protocol of a trial to investigate the effects of over-ground walking (OGW) exercise and CR on cognitive function among stroke survivors. Methods: This is a three-centre, single blind, randomised controlled trial (RCT). Fifty-seven stroke survivors with mild to moderate cognitive impairments will be recruited from three healthcare facilities and randomly assigned into three groups. Group A will receive moderate intensity OGW exercise. Group B will receive CR using an adapted cognitive intervention comprising an attention and a memory tasks. Group C will receive combine OGW and CR exercise. All interventions will be thrice weekly for 8 weeks. Outcomes to be assessed at baseline, 4th and 8th week of interventions are cognition, brain-derived neurotrophic factor (BDNF), quality of life and participation using Mini-Mental State Examination, Melsin brand ELISA kit, stroke-specific quality of life and London Handicap Scale, respectively. Friedman and Kruskal–Wallis tests will be used for within-group and between-group comparison of data. Discussion: This trial is the first to examine the effect of OGW exercise on cognition and BDNF among stroke survivors. 1 results 1
- Background: In order to facilitate case identification of incident (untreated and recent onset) cases of psychosis and controls in three sites in India, Nigeria and Trinidad, we sought to understand how psychoses (or madness) were conceptualized locally. The evidence we gathered also contributes to a long history of research on concepts of madness in diverse settings. Methods: We conducted focus group discussions and individual interviews to collect information about how informants in each site make sense of and respond to madness. A coding framework was developed and analyses of transcripts from the FGDs and interviews were conducted. Results: Analyses suggest the following: a) disturbed behaviors are the primary sign of madness; b) madness is attributed to a wide range of causes; and, c) responses to madness are dictated by cultural and pragmatic factors. These findings are congruent with similar research that has been conducted over the past 50 years. Conclusions: The INTREPID research suggests that concepts about madness share similar features across diverse settings: a) terms for madness are often derived from a common understanding that involves disruptions in mental processes and capacities; b) madness is recognized mostly by disruptive behaviours or marked declines in functioning; c) causal attributions are varied; and, d) help-seeking is a complex process. 1 results 1
