ENGLISH SUMMARY: ABSTRACT Introduction: Community Health Workers are the first point of contact of a community with the primary health care system, so it is important that they possess adequate nutrition knowledge, and a positive attitude to effectively provide nutrition services to their respective communities. Objectives: 1) To determine the nutritional knowledge, attitudes and practices of trained rural health motivators (RHMs); 2) to assess their perceptions and how they apply their nutritional knowledge in their work as RHMs and 3) to assess RHMs trainers’ perceptions of the nutrition training programme as an empowering tool. Study design: A cross-sectional descriptive study design was utilised and a mixed methods approach was employed where quantitative and qualitative data were collected. Methods: For the quantitative part of the study 141 rural health motivators (RHMs) were recruited from six constituencies in the Shiselweni region, Eswatini. Data on demographics, knowledge, attitudes and practices was collected using the self-administered questionnaire. Qualitative data was collected using focus group discussions (FGDs) and in-depth interviews (IDIs). There were 28 FGDs participants and the purpose of the FGDs was to explore the perceptions, opinions and experiences of RHMs, while IDIs were done with all the six Shiselweni region RHM trainers who were key informants. Results: The participants’ nutrition knowledge varied from 48% (n=68) of RHMs having an adequate nutrition knowledge, to 37% (n=52) scoring average, while 15% (n=21) got poor results. The overall attitude of participants regarding the importance of good nutrition was found to be positive, whilst some had a negative attitude towards certain issues that were discussed. In general nutrition practices of participants were very poor, regardless the nutritional knowledge they possessed. Eight seven percent knew the different food groups and their sources, and 57% (n=80) did not have their own backyard gardens as a source of micronutrients. Consumption of fruit and vegetables was low, since 61% (n=86) only ate fruit once a day and 73%(n=103) ate vegetables once a day, which was inadequate, and only 19.9% (n=28) drank 2 litres or more of water, which is the daily recommended intake. The knowledge of participants was fairly good, however their practices did not correlate with the level of nutrition knowledge. The RHMs’ trainers used seminars, demonstrations and nutrition campaigns to train the RHMs and they highlighted that the programme was important since RHMs were a vital link between the community and the health care facility therefore, it was important to capacitate them to prevent and manage nutrition related diseases. Conclusion: RHMs highlighted that the nutrition programme was beneficial to them despite the challenges experienced, because they acquired skills and knowledge that has benefitted them as individuals, their families and communities. RHM trainers recommended that the Government of Eswatini should fully support the programme through provision of the essential budget, personnel, timely training sessions, supportive supervision and incentives.
