Faculty of Medicine and Health Sciences. Dept. of Nursing & Midwifery. Hospitals -- Accreditation -- Saudi Arabia Nurses -- Attitudes -- Saudi Arabia Medical care -- Evaluation -- Saudi Arabia UCTD Thesis (MNur)--Stellenbosch University, 2021. ENGLISH SUMMARY : Worldwide, accreditation is considered an important tool to improve quality care and patient safety in healthcare organizations, where in Saudi Arabia, hospitals are mandated to comply with the national accreditation standards. Although nurses have a major role in implementing the accreditation standards, little is known about their perception of the impact of hospital accreditation on quality of care. Methods: The aim of the study was to explore and describe nurses` perception on the quality of care related to national and international accreditation in an armed forces hospital in Saudi Arabia. The objectives of the study were to: 1) to explore and describe nurses` perspectives on the impact of the hospital accreditation on improving quality care and patient safety and 2) to make recommendations about the perspectives of the nurses as stakeholders to maximize the process of accreditation. The structure, process, and outcome domains of Donabedian’s conceptual model for quality care provision were employed to explore the nurses’ perception of national and international accreditation on quality of care in an armed forces hospital in Saudi Arabia. This study used a quantitative cross-sectional descriptive survey design. Data was collected using a self-administered questionnaire survey instrument which included nine sections and sub-sections rated on five-point Likert response scale. A systematic random sampling method was used to select the participants of this study. Results: A total of 116 respondents participated in this study, with an effective response rate of 94%, of which most of the respondents were female at (n=111) 95.5%. The survey tool presented a strong internal consistency, demonstrated by the subscales’ Cronbach‘s alpha values of more than 0.83. The Pearson correlation measurement indicated that the relationship between the quality process variables and the quality outcomes variable were all positive. The correlation with the quality results was the strongest with the variable of ‘use of data’, and the lowest with the variable ‘quality measurement and analysis’. A stepwise regression model indicated that the accreditation quality process aspect of ‘use of data’ had the most significant association with better quality outcomes. However, no significant impact resulted from the other quality process variables of ‘quality measurement and analysis’, ‘leadership commitment and support’, and ‘benefits of accreditation’ on quality results. Conclusion: Nurses in this study perceived the accreditation quality process variables of ‘use of data’, ‘staff involvement’, ‘human resources education and training’, and ‘quality management’ to be significantly associated with better quality outcomes. However, quality outcomes were not associated with the accreditation quality process variables of ‘leadership commitment and support’, ‘quality measurement and analysis’, and ‘benefits of accreditation’. This could indicate the value of sustaining the improvement after hospital accreditation recognition by the continuous use of data, staff involvement in the accreditation process, human resources education and training to help with quality management, to maintain quality outcomes post accreditation.
