ENGLISH ABSTRACT: Introduction: Childhood malnutrition continues to be a major challenge in South Africa, where severe malnutrition is a contributor in almost a third of deaths under the age of five, and the prevalence of under-five growth stunting has remained static at around 27% since 1999. The success of Community-based Therapeutic Care (CTC) in reducing mortality from severe acute malnutrition is well documented, although the sustainability of shortterm successes is unknown. Modelled on CTC principles, the Nutrition Therapeutic Programme (NTP) is well established in the Western Cape in South Africa, but outcomes of this six-month programme remain largely unknown. The aim of our study was to determine outcomes of malnourished children after 6 months on the NTP in terms of successful treatment, weight gain, defaulting rates and mortality, and to compare these outcomes to international Sphere standards. Methods: A retrospective descriptive cohort study was conducted in a peri-urban, lowincome settlement in the Western Cape. 270 patients meeting criteria for malnutrition between the age of 6 months and 5 years were recruited from 3 community clinics from January 2017 – December 2018. Results: Outcomes revealed a mortality rate of 0,7%, which was below the international Sphere target of 75%), and average daily weight gain at 6 months of 0,76g/kg/day (target >3g/kg/day). Possible risk factors for a poor outcome in this study included HIV positive status, older age, enrollment during winter or springtime and management by a nurse versus a dietitian. Although patient weights were very well documented, other indices were poorly documented. Our results suggest a very high rate of measurement error for height with 41% of documented baseline height-for-age values falling in the WHO defined “biologically implausible” range. Conclusion: We concluded that Sphere targets may not be appropriate success metrics in this context, as the increased length of the programme may be responsible for the higher defaulting rates, whilst recourse to in-hospital treatment means lower mortality rates are achievable. The Western Cape NTP guidelines are due to be updated soon. We feel that clear, achievable, and tailored local targets should be set in order to keep healthcare practitioners motivated and accountable.
