ENGLISH SUMMARY: Background: There is limited literature about circulating nurses in the Operating Room especially within the South African context. In the South African context, Enrolled Nursing Assistants or Enrolled Nurses generally fulfil the role of the circulating nurse in the OR, whereas internationally this role is typically filled by a Professional Nurse due to the critical importance thereof. Circulating nurses require impeccable procedural knowledge to anticipate unpredictable, often life-threatening, risks in the OR. This begs the question of whether a nurse with a one - or two-year qualification is able to fulfil this role. It is important to understand how OR staff experience the role and behaviour of circulating nurses to inform strategies aimed at improving perioperative patient safety and comprehensive care delivered by the surgical team. Research Question: This study was guided by the question: “How do operating room staff experience the role and behaviour of circulating nurses in the Operating Room?” Aim: The aim of this study was to explore the experience that operating room staff have of the role and behaviour of circulating nurses, in private healthcare facilities in the Western Cape. Objectives: The objectives of the study were to understand the experiences that operating room staff have of the role and behaviour of circulating nurses, in the perioperative environment, with specific reference to the following objectives: · To understand how operating room staff experience the role of circulating nurses. · To understand how operating room staff experience the behaviour of circulating nurses. Research Process: A descriptive qualitative approach was applied in this study. Prior to the study, ethical approval was obtained from the Health Research Ethics Committee of Stellenbosch University and the private healthcare organisation in the Western Cape. Data was collected through recorded individual semi-structured interviews with nineteen purposefully selected participants. The anonymity of all participants was ensured through the allocation of pseudonyms. The interviews were transcribed verbatim, and analysed using Creswell’s data analysis steps. Lincoln and Guba’s criteria were applied throughout the process, in order to ensure the trustworthiness of the study. Results: This study found that the role of circulating nurses is experienced differently by various surgical team members. Whilst all participants related circulating nurses ’role to their function within a high -tech, pressured, specialised environment, they had diverse expectations from this role. Participants agreed on the importance of this role in the OR, although some OR staff held circulating nurses in very low regard, viewing them as workhorses, rather than contributors to comprehensive perioperative patient care. This study further found that participants’ experience of the competency of circulating nurses was related to their skills, knowledge, and training, although various OR team members had different perceptions about the educational level or qualifications of circulating nurses. Circulating nurses in this study regarded themselves as valued team members. However, many participants related the value of circulating nurses as team members to their ability to work fast, learn quickly, and communicate effectively without cutting corners. Conclusion: The study emphasized the diverse role expectations and questionable behaviour in respect of the circulating role. With that as background, the OR staff experienced that the behaviour of circulating nurses influenced their competence in the role. AFRIKAANSE OPSOMMING: Agtergrond: Daar is beperkte literatuur oor vloerverpleegsters in die operasiesaal veral binne die Suid-Afrikaanse konteks. In die Suid-Afrikaanse konteks vervul Ingeskrewe Verpleeghulp (ENA's), of Ingeskrewe Verpleegsters (EN's), oor die algemeen die rol van die vloerverpleegster in die operasiesaal, terwyl hierdie rol internasionaal tipies deur 'n Professionele Verpleegster (PN) gevul word, as gevolg van die kritieke belang daarvan. Daar word van vloerverpleegsters verwag om onberispelike prosedurekennis te he om te voorsien vir onvoorspelbare, dikwels lewensgevaarlike risiko's in die operasiesaal. Dit laat die vraag ontstaan of 'n verpleegkundige, wat op 'n laer vlak gekwalifiseer is, in staat is om hierdie rol te vervul. Dit is belangrik om te verstaan hoe teaterpersoneel die rol en gedrag van vloerverpleegsters ervaar om strategiee in te lig om perioperatiewe pasient-veiligheid en omvattende sorg deur die chirurgiese span te verbeter. Navorsingsvraag: Hierdie studie is gelei deur die vraag: "Hoe ervaar teaterpersoneel die rol en gedrag van sirkulerende verpleegsters in die operasiesaal?" Doel: Die doel van hierdie studie was om die ervaring wat teaterpersoneel het van die rol en gedrag van vloerverpleegsters in Privaat Gesondheidsorgfasiliteite in die Wes-Kaap te verken. Doelwitte: Die doelwitte van die studie was om die ervarings wat teaterpersoneel het van die rolle en gedrag van vloerverpleegsters in die perioperatiewe omgewing te verstaan, met spesifieke verwysing na die volgende doelwitte: • Om te verstaan hoe teaterpersoneel die rol van vloerverpleegsters ervaar. • Om te verstaan hoe teaterpersoneel die gedrag van vloerverpleegsters ervaar. Navorsingsproses: 'n Beskrywende kwalitatiewe benadering is in hierdie studie toegepas. Etiese goedkeuring is gekry van die Gesondheidsnavorsingsetiekkomitee (HREC) van die Universiteit Stellenbosch en die private gesondheidsintelling in die Wes-Kaap, voor die studie. Data is ingesamel deur opgeneemde individuele semi-gestruktureerde onderhoude met negentien (19) doelgerig geselekteerde deelnemers. Die anonimiteit van alle deelnemers is verseker deur die toekenning van skuilname. Die onderhoude is woordeliks getranskribeer, en geanaliseer met behulp van Creswell se data-ontledingstappe. Lincoln en Guba se kriteria is deurgaans toegepas om die betroubaarheid van die studie te verseker. Resultate: Die studie het bevind dat die rol van vloerverpleegsters verskillend deur verskeie teaterpersoneel ervaar word. Alhoewel alle deelnemers vloerpleegsters se rol in verband gebring het met hul funksie binne 'n hoe-tegnologiese, hoe-druk, gespesialiseerde omgewing, het hulle uiteenlopende verwagtinge van hierdie rol gehad. Deelnemers het saamgestem oor die belangrikheid van hierdie rol in die operasiesaal, alhoewel sommige teaterpersoneel vloerverpleegsters met baie lae aansien beskou het en hulle as werkesels beskou het, eerder as bydraers tot omvattende perioperatiewe pasientsorg. Hierdie studie het verder bevind dat deelnemers se ervaring van die bevoegdheid van vloerverpleegsters verband hou met hul vaardighede, kennis en opleiding, hoewel verskeie teaterpersoneel verskillende persepsies gehad het oor die opvoedingsvlak of kwalifikasies van vloerverpleegsters. Vloerverpleegsters in hierdie studie het hulself as gewaardeerde spanlede beskou, soos uitgespreek deur een verpleegster: "'n Suster het altyd vir ons gese dat ons belangrik is, en dat jy nie jouself moet ondermyn nie, of voel dat jy net 'n vloerverpleegster is nie, maar dat jy ‘n groter rol speel” (AN 1). Baie deelnemers het egter die waarde van vloerverpleegsters as 'n spanlid in verband gebring met hul vermoe om vinnig te werk, vinnig te leer en effektief te kommunikeer sonder om hoeke te sny. Gevolgtrekking: Die studie het uiteenlopende rolverwagtinge en twyfelagtige gedrag van die rol van die vloerverpleegster beklemtoon, alhoewel teaterpersoneel ervaar dat die gedrag van vloerverpleegsters hul bevoegdheid in die rol beinvloed.
