ENGLISH SUMMARY: Introduction: Type 2 diabetes mellitus (T2DM) is one of many chronic illnesses, and it is a condition that requires a good health literacy level to be able to implement self-care behaviours that can contribute to control and maintain optimal blood glucose. Diabetes self-management education (DSME) has demonstrated that it is effective in improving several health outcomes of individuals suffering from T2DM. In Zambia, DSME has not been adopted to function according to guidelines, as most low-income countries like Zambia still depend on the education provided by the doctor’s office to support T2DM self-care. Objective: This study aimed to assess the efficacy of a DSME programme for the health literacy of patients with T2DM attending one of the first-level hospitals in the Lusaka District of Zambia. Method: In the study design, a quasi-experiment one group pre-test and post-test as well as a multistage sampling technique were applied. The total sample size was 242, consisting of T2DM patients attending the Matero Level One Hospital. A four-week DSME programme was provided to 242 T2DM patients, who were allowed to practise the health literacy for an additional four weeks. The study collected data at two points in time, namely before and after the interventions. It assessed the patients’ level of knowledge, attitudes and practices (KAP) regarding T2DM before and after the intervention by administering a structured questionnaire. Data were analysed using proportional Chi-square calculation for correlations, the Wilcoxon Signed Rank Test to test hypotheses, and multinomial logistic regression to assess factors contributing to the KAP of T2DM patients. Results: The median KAP score before intervention was 30, and after intervention 50; the change was significant at a p-value of 0.000 (Wilcoxon Sign Rank Test). The Chi-square correlation revealed that only household income had a correlation to the health literacy of T2DM-patients (p-value 0.000). High KAP level scores increased from 16.9% (n = 41) before intervention to 59.9% (n = 145) after intervention. From the multinomial logistic regression the adequacy of KAP and other outcomes in this study were significantly influenced by several factors including education level, average household income, age, gender, at household income and area of residency affected KAP level at pre intervention, while at post intervention, only marital status, household income, gender, tertiary education influenced both the good level of practice (p-value 0.000) and a positive level of practice (p-value 0.000) relative to a negative level of practice. Conclusion: The study rejected the null hypothesis and concludes that DSME is effective in improving the KAP of T2DM patients, thus increasing the health literacy of the T2DM patients. Recommendations: The T2DM DSME should be provided by dedicated educators soon after T2DM has been diagnosed, and different approaches should be applied to reach different socioeconomic groups effectively. The content should include a focus on promoting local foods to address the diet challenges faced by T2DM patients. The DSME should be allocated adequate time and should be provided regularly following a diagnosis of T2DM. AFRIKAANSE OPSOMMING: Inleiding: Tipe 2-diabetes mellitus (T2DM) is een van vele chroniese siektes, en dit is 'n toestand wat 'n goeie gesondheid-geletterdheidsvlak vereis om selfversorgingsgedrag te kan aanneem wat kan bydra om optimale bloedglukose te beheer en te handhaaf. Selfbestuur-opvoeding vir diabetes (SBOD) het getoon dat dit effektief is om verskeie gesondheidsuitkomste van individue wat aan T2DM ly, te verbeter. In Zambie is SBOD nie aangepas om volgens riglyne te funksioneer nie, aangesien die meeste laeinkomstelande soos Zambie steeds afhanklik is van die onderwys wat deur die dokter se kantoor verskaf word om T2DM-selfsorg te ondersteun. Doelwit: Hierdie studie was daarop gemik om die doeltreffendheid van 'n SBOD-programme vir die gesondheidsgeletterdheid van pasiente met T2DM wat een van die eerstevlak-hospitale in die Lusaka-distrik van Zambie bywoon, te bepaal. Metode: In die studie-ontwerp is 'n voor- en natoets sowel as 'n multi-stadium steekproeftegniek toegepas. Die totale steekproefgrootte was 242, bestaande uit T2DM-pasiente wat die Matero Vlak Een-hospitaal bygewoon het. 'n Vier weke lange SBOD-programme is aan 242 T2DM-pasiente verskaf, wie toegelaat is om die gesondheidsgeletterdheid vir 'n bykomende vier weke te beoefen. Die studie het data op twee tydstippe ingesamel, naamlik voor en na die intervensies. Dit het die pasiente se vlak van kennis, houdings en praktyke (KHP) ten opsigte van T2DM voor en na die intervensie geassesseer deur 'n gestruktureerde vraelys toe te pas. Data is ontleed deur gebruik te maak van proporsionele Chi-kwadraat berekening vir korrelasies, die Wilcoxon Signed Rank Test om hipoteses te toets, en veeltermige logistiese regressie om faktore wat tot KHP van T2DM-pasiente bydra, te assesseer. Resultate: Resultate: Die mediaan KAP-telling voor intervensie was 30, en na intervensie 50; die verandering was betekenisvol teen 'n p-waarde van 0,000 (Wilcoxon Sign Rank Test). Die Chi-kwadraat-korrelasie het aan die lig gebring dat slegs huishoudelike inkomste 'n korrelasie gehad het met die gesondheidsgeletterdheid van T2DM-pasiente (p-waarde 0,000). Hoe KAP-vlaktellings het toegeneem van 16.9% (n = 41) voor intervensie tot 59.9% (n = 145) na intervensie. Vanuit die multinomiale logistieke regressie is die toereikendheid van KAP en ander uitkomste in hierdie studie beduidend beinvloed deur verskeie faktore, insluitend opvoedingsvlak, gemiddelde huishoudelike inkomste, ouderdom en geslag by vasgestel dat huishoudelike inkomste en area van verblyf KAP-vlak by voor-intervensie beinvloed het, terwyl by na-intervensie het slegs huwelikstatus, huishoudelike inkomste, geslag, tersiere opleiding beide die goeie vlak van praktyk (p-waarde 0.000) en 'n positiewe vlak van praktyk (p-waarde 0.000) beinvloed relatief tot 'n negatiewe vlak van praktyk. Gevolgtrekking: Die studie het die nulhipotese verwerp en tot die gevolgtrekking gekom dat SBOD effektief is in die verbetering van die KHP van T2DM pasiente, en sodoende die gesondheidsgeletterdheid van die T2DM-pasiente verhoog. Aanbevelings: Die T2DM SBOD behoort deur toegewyde opvoeders verskaf te word kort nadat T2DM gediagnoseer is, en verskillende benaderings moet toegepas word om verskillende sosio-ekonomiese groepe effektief te bereik. Die inhoud moet 'n fokus op die bevordering van plaaslike voedsel insluit om die dieetuitdagings wat T2DM-pasiente in die gesig staar, aan te spreek. Voldoende tyd moet aan die SBOD toegeken word en moet na 'n diagnose van T2DM gereeld verskaf word.
