ENGLISH SUMMARY: Background: Elderly persons are vulnerable, often dependent on caregivers, and highly susceptible to malnutrition. Malnutrition can be defined as insufficient nutritional intake or absorption to maintain body weight or fat-free mass. Worldwide, the risk of malnutrition in the elderly is between 38 and 60% and malnutrition prevalence between 8 and 21%. Residents of old age homes are especially dependent on caregivers for nutritional care. Insufficient knowledge and negative attitudes regarding nutrition are barriers to adequate nutrition. Inadequate nutritional care can increase the prevalence of malnutrition among residents, decrease their quality of life, and place a heavy burden on healthcare systems. Previous studies found that caregivers' knowledge of nutrition varies, and their attitudes towards nutrition are mostly neutral. The aim of this study was to determine the prevalence of malnutrition in old age home residents, and to determine the attitudes and knowledge old age home caregivers have regarding nutrition for residents in the Langeberg district in South Africa. Methods: This study was a cross-sectional observational study with an analytical component. Consenting residents of eight old age homes in the Langeberg district were recruited to the study using a combination of random and convenience sampling. Data collected included demographic data and assessment for the prevalence of malnutrition or risk thereof using the validated Nestlé Mini Nutritional Assessment (MNA®). Consenting caregivers permanently employed at the old age homes were also randomly recruited to the study. Data collected included demographic data, an assessment of their knowledge of nutrition using the validated Knowledge of Malnutrition – Geriatric 2.0 Questionnaire (KOM-G 2.0), and an assessment of their attitudes using the validated Staff Attitudes to Nutritional Nursing Geriatric Care scale (SANN-G). Descriptive and inferential statistics were performed by a statistician using STATA 17 software. A p value of less than 0.05 was considered significant. Results: One hundred and four residents were included in the study: 6.7% were malnourished, 30.8% at risk of malnutrition, and 62.5% well nourished. The average age of residents was 83 years, their average BMI was 25kg.m-2 and the average score on the MNA was 24, therefore mostly well nourished. Seventy-seven caregivers were included in the study. There were 62 carers (81%), four (5%) enrolled nurses, one (1%) nursing assistant, six (8%) registered nurses, and four (5%) trainee carers. Their mean score on the KOM-G 2.0 was 10 (SD 2.84) out of 16, indicating 63% correct answers. Caregivers scored lower on questions relating to timing and frequency of nutritional screening, with only 28 (36%) correct answers, and elderly fluid requirements with 27 (35%) correct answers. Caregivers' mean score on the SANN-G was 49.68 (SD 13), indicating negative attitudes. No significant correlation was found between attitudes and knowledge of caregivers. Conclusion: Malnutrition and the risk thereof is prevalent in old age homes in the Langeberg district. Caregivers have good knowledge, but negative attitudes regarding nutrition. Potential interventions to reduce the risk of malnutrition among residents of old age homes could be implemented, with specific focus on hydration and malnutrition screening protocols. Further training of caregivers focusing specifically on screening for malnutrition and on hydration could be invaluable. AFRIKAANSE OPSOMMING: Agtergrond: Bejaarde persone is weerloos, baie keer afhanklik van versorgers en hoogs vatbaar vir wanvoeding. Wanvoeding kan gedefinieer word as onvoldoende inname of absorpsie van voeding om liggaamsgewig en vetvrye massa te handhaaf. Die risiko van wanvoeding onder bejaardes wissel wereldwyd tussen 38 en 60% en die voorkoms van wanvoeding tussen 8 en 21%. Inwonwers van ouetehuise is afhanklik van sorgpersoneel vir voedingsorg. Gebrekkige kennis en negatiewe ingesteldheid van sorgpersoneel teenoor voeding kan die kwaliteit van voedingsorg negatief beinvloed. Onvoldoende voedingsorg kan 'n hoer voorkoms van wanvoeding onder inwoners teweeg bring, hul kwaliteit van lewe benadeel en die las op die gesondheidstelsel verhoog. Vorige studies het bevind dat sorgpersoneel se kennis rondom voeding varieer en dat hul ingesteldheid meestal neutraal is. Die doelwit van hierdie studie was om die voorkoms van wanvoeding onder ouetehuis inwoners en die kennis en ingesteldheid van sorgpersoneel met betrekking tot voeding in die Langeberg distrik in Suid-Afrika vas te stel. Metodes: Hierdie studie was 'n deursnit waarnemende studie met 'n analitiese komponent. Instemmende inwoners van ag ouetehuise in die Langeberg distrik is gewerf om deel te neem aan die studie deur 'n kombinasie van ewekansige en geriefsteekproefneming. Demografiese data is ingesamel en 'n assessering vir wanvoeding, of risiko vir wanvoeding, is gedoen met Nestle se "Mini Nutritional Assessment" (MNA). Instemmende sorgpersoneel wat permanent aangestel is by ouetehuise is ook ewekansig genooi om deel te neem aan die studie. Demografiese data is ingesamel, hul kennis is geassesseer met die "Knowledge of Malnutrition – Geriatric 2.0" (KOM-G 2.0) vraestel, en hul ingesteldheid is geassesseer met die "Staff Attitudes to Nutritional Nursing Geriatric Care" (SANN-G) vraestel. Beskrywende statistiek en inferensie is deur 'n statistikus gedoen met STATA 17 sagteware. 'n P waarde van minder as 0.05 is beskou as betekenisvol. Resultate: Een honderd en vier inwoners is ingesluit in die studie, waarvan 6.7% wangevoed was, 30.8% in gevaar van wanvoeding was en 62.5% goed gevoed was. Die inwoners se gemiddelde ouderdom was 83 jaar, hul gemiddelde liggaamsgewig indeks was 25kg.m-2 en hul gemiddelde resultaat van die MNA was 24, dus goed gevoed. Sewe en sewentig sorgpersoneel lede is ingesluit in die studie, onder wie 62 versorgers (81%), vier (5%) ingeskrewe verpleegkundiges, een (1%) verpleegassistent, ses (8%) geregistreerde verpleegkundiges en vier (5%) leerling versorgers. Hul gemiddelde resultaat in die KOM-G 2.0 was 10 (SD 2.84) uit 16, dus 63% korrekte antwoorde. Sorgpersoneel het minder korrekte antwoorde gehad op vrae oor die tydsberekening van siftingsondersoeke vir voedingstatus met slegs 28 (36%) korrekte antwoorde en vloeistof benodiging van bejaardes met slegs 27 (35%) korrekte antwoorde. Sorgpersoneel se gemmiddelde telling van die SANN-G was 49.68 (SD 13), dus negatiewe ingesteldheid teenoor voeding. Geen betekenisvolle korrelasie is gevind tussen die kennis en ingesteldheid teenoor voeding onder sorgpersoneel nie. Gevolgtrekking: Daar is risiko vir wanvoeding en voorkoms van wanvoeding in ouetehuise in die Langeberg. Sorgpersoneel het goeie kennis, maar negatiewe ingesteldhede teenoor voeding. Die risiko en voorkoms van wanvoeding kan moontlik verminder word deur die implementering van ingrypings met spesifieke fokus op siftingsondersoeke vir voedingstatus en hidrasie. Verdere opleiding van sorgpersoneel oor siftingsondersoeke en hidrasie vir bejaardes mag van waarde wees.
