However, mindfulness research is lacking in low socio-economic contexts. This study explored how an MBP may be adapted to benefit participants' well-being in a low socio-economic setting in Cape Town, South Africa. Methods: The study was conducted in two phases. In Phase 1, a qualitative descriptive design was used. Fourteen stakeholders (mindfulness professionals, The Big Issue staff and The Big Issue vendors) were identified using purposive and snowball sampling. The stakeholders participated in semi-structured interviews and focus groups. The data were thematically analysed. In Phase 2, the researcher developed a manual to guide mindfulness-based programme designers and facilitators in adapting mindfulness programmes for low socio-economic settings. Findings: Stakeholders agreed that MBPs could be adapted to increase acceptability, accessibility and feasibility for participants. The themes that emerged included strategies to encourage attendance and retention, program structure, program content, trauma sensitivity, the qualities and training of the facilitator, communication strategies, the language used, and the approach to translation. A manual was developed based on the findings. Conclusion: Stakeholders agreed that adapted MBPs may be beneficial in low socio-economic contexts to enhance participants' sense of well-being. The success of the MBP will depend on the skills and qualities of facilitators and the time and place when the sessions are held. Further research is needed into the nuances of what, where, when, and how MBPs are offered in low-resource and especially high-risk trauma environments, such as those in South Africa.
