Die literatuur wys egter daarop dat die land se spraak-taalterapeute 'n klein, homogene groep is wat hoofsaaklik uit Engels-en Afrikaanssprekende vroue bestaan. Daarby is hulpbronne en riglyne vir kultuurresponsiewe kruislinguistiese en-kulturele vroee kommunikasie-intervensie beperk. Hierdie proefskrif beskryf 'n diepgaande ondersoek na kruislinguistiese en -kulturele vroee kommunikasie-intervensie. Die ondersoek handel in die besonder oor: • die aard en omvang van die bestaande literatuur oor kruislinguistiese en -kulturele pediatriese spraak-taalterapie; • die ervarings van spraak-taalterapeute en Xhosa-sprekende versorgers in die Oos-Kaap; en • die ontwikkeling en beoordeling van 'n oorspronklike dokument met riglyne om kultuurresponsiewe praktyke gedurende kruislinguistiese en -kulturele vroee kommunikasie-intervensie te bevorder. Metodologie: Die navorsing is in Vier studiefases onderneem. Studie 1 was 'n omvangsbepaling wat die landskap van bestaande internasionale literatuur oor kruislinguistiese spraak-taalterapie vir kinders gekarteer het. Die omvangsbepalingsmetodologie van JBI (voorheen "die Joanna Briggs instituut") is as metodologiese raamwerk gebruik. In studie 2 en 3 is hermeneuties-fenomenologiese onderhoude met onderskeidelik ses spraak-taalterapeute en sewe versorgers gevoer. Die data van studie 2 en 3 is daarna volgens Ricoeur se teorie ontleed en vertolk. In studie 4 is 'n oorspronklike dokument met riglyne vir kruislinguistiese en-kulturele vroee kommunikasie-intervensie ontwikkel, wat 41 belanghebbendes uiteindelik aan die hand van 'n aanlyn vraelys met 18 items beoordeel het. Beskrywende en inferensiele statistiese metodes is gebruik om die numeriese data uit die vraelys te ontleed, terwyl die antwoorde op die oop vrae tematies ontleed is. Bevindinge en bespreking: In studie 1 het 12 artikels aan die kriteria vir insluiting by die omvangsbepaling voldoen. Die studies is meestal na 2015 in veral die Verenigde State en Kanada uitgevoer en in verskillende vaktydskrifte gepubliseer. Semigestruktureerde onderhoude en vraelyste was die mees gebruikte data-insamelingsmetodes. Die artikels het kommunikasie-afbrekings, probleme met tolke, samewerking deur die versorger sowel as 'n tekort aan kultuurgepaste materiaal vir assessering en intervensie as struikelblokke uitgelig. Aangemelde strategiee het ingesluit om die versorger se samewerking en betrokkenheid te probeer verseker en om die versorger se kultuur, waardes, geloofsoortuiging en persepsie oor die kind se kommunikasieprobleem te respekteer. In studie 2 is bepaalde temas uit die deelnemers se vertellings ontwikkel, naamlik 'n ervaring van innerlike wroeging, 'n bewustheid van hulle eie kultuurbevoegdheid, die belang van ervaring, struikelblokke waarvoor hulle te staan kom, en verskillende strategiee om daardie struikelblokke te oorkom. Die vertellings van die versorgerdeelnemers aan studie 3 het weer klem gele op die waarde van inligting oor spraak-taalterapie, die profiel van die spraak-taalterapeute wat hulle bedien, die doeltreffendheid van vroee kommunikasie-intervensie, versorgers se verwagtinge, deelname en rol, en die waarde wat die deelnemers aan hulle moedertaal en kultuur heg. In studie 4 is 'n oorspronklike dokument met riglyne en aanbevelings vir kruislinguistiese en-kulturele vroee kommunikasie-intervensie ontwikkel op grond van die bevindinge van studie 1 tot 3. Die meerderheid (95%) van die belanghebbendes wat aan studie 4 deelgeneem het, het saamgestem dat die riglyne uitvoerbaar en van 'n hoe gehalte is. Gevolgtrekking: 'n Deeglike kennis van spraak-taalterapeute en versorgers se ervarings het die grondslag gele vir die formulering van riglyne wat spraak-taalterapeute kan ondersteun deur die gaping tussen ideale en werklike praktyk vir kultuurresponsiewe vroee kommunikasie-intervensie te vernou. ENGLISH SUMMARY: Background: South African speech-language therapists are expected to provide quality evidence-based early communication intervention to caregivers and their children, irrespective of the caregiver's linguistic and cultural background. However, the literature indicates that the country's speech-language therapists are a small, homogenous group, mainly comprised of English-speaking and Afrikaans-speaking women. Moreover, resources and guidelines for culturally responsive cross-linguistic and cross-cultural early communication intervention are severely limited. This dissertation describes an in-depth study into cross-linguistic and cross-cultural early communication intervention. The study is specifically concerned with: • the nature and scope of the existing literature on cross-linguistic and cross-cultural paediatric speech-language therapy; • the experiences of speech-language therapists and Xhosa-speaking caregivers in the Eastern Cape; and • the development and review of an original guideline document aimed at promoting culturally responsive practices during cross-linguistic and cross-cultural early communication intervention. Methodology: The research was conducted in four study phases. Study 1 comprised a scoping review that mapped the landscape of existing global literature on cross-linguistic speech-language therapy for children. The scoping review methodology of JBI (formerly "the Joanna Briggs Institute") served as methodological framework. In studies 2 and 3, hermeneutic phenomenological interviews were conducted with six speech-language therapists and seven caregivers respectively. The data from studies 2 and 3 were subsequently analysed and interpreted in accordance with Ricoeur's theory of interpretation. In study 4, an original document with guidelines for cross-linguistic and cross-cultural early communication intervention was developed, which 41 stakeholders eventually reviewed by completing an 18-item online questionnaire. Descriptive and inferential statistical methods were used to analyse the numerical data from the questionnaire, while the responses to the open-ended questions were analysed thematically. Findings and discussion: In study 1, 12 articles met the criteria for inclusion in the scoping review. These studies were mostly conducted in the United States and Canada after 2015 and were published in various journals. Semi-structured interviews and questionnaires were the most commonly used data collection methods. The articles highlighted barriers such as communication breakdowns, problems with interpreters, caregiver buy-in, and a lack of culturally appropriate material for assessment and intervention. Strategies reported included attempts to ensure the caregiver's cooperation and engagement, and respect for the caregiver's culture, values, religious convictions, and perception of the child's communication difficulty. In study 2, certain themes were developed from the participating speech-language therapists' stories, namely a sense of inner struggle, an awareness of their own cultural competence, the importance of experience, the stumbling blocks they face, and various strategies for overcoming those stumbling blocks. The stories of the participating caregivers in study 3, in turn, emphasised the value of information on speech-language therapy, the profile of their attending speech-language therapists, the effectiveness of early communication intervention, caregivers' expectations, buy-in and role, and the value that participants attached to their mother tongue and culture. In study 4, the findings from studies 1 to 3 served as the basis for developing an original document with guidelines and recommendations for cross-linguistic and cross-cultural early communication intervention. The majority (95%) of the stakeholders who participated in study 4 agreed that the guidelines were feasible and of a high quality. Conclusion: An in-depth knowledge of the experiences of speech-language therapists and caregivers laid the foundation for the formulation of guidelines that could support speech-language therapists by narrowing the gap between ideal and actual practice for culturally responsive early communication intervention.
