Available: https://scholar.sun.ac.za/items/7ac05f3e-eb0f-45e9-b281-f7a1dfae4e27 ENGLISH ABSTRACT: Background: Iatrogenic visceral injury at Caesarean section (CS) is associated with significant morbidity and is largely preventable if cautionary measures are taken in high risk patients. Objective: To describe the risk factors and outcomes of women who obtained visceral organ injuries during caesarean section (CS) at Tygerberg Hospital (TBH) in Cape Town. Methods: We conducted a retrospective descriptive study of women who sustained visceral organ injury at CS. Theatre registers were screened to identify cases between January 2019 and December 2023. Results: In the 5-year study period, 20163 CS’s were performed, and 65(0.32%) patients were identified as having a visceral organ injury. The injuries sustained were bladder 42(64.6%), bowel 15(23%) and ureteric 8(12.3%). The mean age was 32(SD5), 39(60%) had a Body mass index (BMI) of >30kg/m2 with a mean delivery gestation of 37 weeks(SD3.3). There were 49(75%) who had at least one previous CS, which was more common in women who sustained bladder injuries, 37(88.10%). The majority were emergency CS 47(72%), and in 40(65%) of cases, the women were in labour. A blood loss of >1000ml was identified in 30(46,15%), including 7 of the 8 women who sustained ureteric injuries. Eight(12,31%) of patients required admission to an intensive care unit, and there were two maternal deaths, both sustained bowel injuries, with delayed recognition. Conclusion: This study shows that the incidence of visceral organ injury sustained at CS at TBH is low. Bladder injuries were associated with previous CS and ureteric injuries with postpartum haemorrhage. Women who sustained bowel injuries had the highest mortality. Delays in recognition of such injuries carry catastrophic risks; meticulous intra- and post-operative practices should be in place to avoid such incidents.
