A Retrospective Audit of Out-Patient Hysteroscopy at Tygerberg Hospital over a Five-Year Period. Unpublished masters thesis. Stellenbosch: Stellenbosch University [online]. Available: https://scholar.sun.ac.za/items/31ddd86a-2579-4e71-ad4b-340939ba2d21 ENGLISH ABSTRACT: Background: Outpatient hysteroscopy is a widely used diagnostic and therapeutic procedure for addressing various intrauterine pathologies, including abnormal uterine bleeding (AUB), postmenopausal bleeding (PMB), and infertility. Its minimally invasive nature reduces the need for general anaesthesia and inpatient care, promoting efficiency and patient comfort. This study presents a retrospective audit of outpatient hysteroscopy procedures at Tygerberg Hospital over a five-year period (2015–2019). Objectives: The primary objective was to evaluate the indications for outpatient hysteroscopy, while the secondary objective focused on assessing the complication rates associated with the procedures. Methods: A retrospective cross-sectional study was conducted using data from 1,966 patients who underwent outpatient hysteroscopy. Key data points included patient demographics, procedural indications, and complications. Statistical analysis was performed to determine the distribution of indications and identify trends in procedural outcomes and safety. Results: The most common indication was PMB (45.27%), followed by AUB (16.33%) and suspected uterine anomalies (10.33%). Other indications included recurrent pregnancy loss (8.09%), polyps (6.05%), and fibroids (4.37%). The procedure was completed successfully in 90.13% of cases, with only 7.32% classified as incomplete, mainly due to cervical stenosis (3.46%) and pain (2.54%). The overall complication rate was low (1.12%), with three cases of perforation and bleeding reported. No fluid overload or infections were observed. Conclusion: Outpatient hysteroscopy at Tygerberg Hospital proved to be a safe and effective procedure with minimal complications. The findings underscore its importance in managing PMB, AUB, and other intrauterine pathologies. Expanding outpatient hysteroscopy services, coupled with enhanced registrar training, can further optimize patient care and resource allocation. Future research should incorporate patient-reported outcomes to provide a more comprehensive evaluation of the service.
