Available: https://scholar.sun.ac.za/items/34a332eb-2bc7-4d7f-ab4a-7058fc6c1fdd ENGLISH SUMMARY: The sustainability of high-risk industries such as mining, oil, gas, and energy depends on their ability to remain profitable while simultaneously maintaining optimal safety performance, among other governance goals. These industries predominantly adopt reliability-based approaches to safety management, typically implemented through compliance-driven safety management systems that aim to reduce behavioural variability among workers. Such systems are designed to create a structured environment within which production goals can be safely pursued, particularly by those responsible for executing or supervising work at the operational level. However, as living, complex, and dynamic socio-technical systems, organisations often exhibit outcomes that are more nuanced than intended. The relationship between reliability-based safety management and safety rule violation behaviour (SRVB) is thus likely to be dynamic and complex. A case in point is the South African mining industry, the focus of this study, which, despite notable safety improvements over recent decades and being one of the most highly regulated sectors in the country, continues to experience serious safety incidents and disasters. Aiming to offer evidence-based insights into these persistent safety challenges, the study pursued two broad objectives. First, the study siught to understand both the value and limitations of reliability-based safety management, through a high-risk industry case study. Second, it sought to explore how resilience-based safety management can be optimally integrated, including the systemic constraints that may hinder its adoption, in a high-risk industrial system. Guided by an interpretivist phenomenological paradigm, the study employed an explanatory single embedded critical case study design, using system dynamics causal loop modelling as its primary analytical method. The findings underscore the dynamic complexity of managing human reliability and safety in high-risk socio-technical systems that also seek to achieve other strategic goals such as productivity and cost control. Specifically, the study illustrates how strategies and factors aimed at achieving these other goals can, in complex and often unintended ways, contribute to the very safety rule violations that organisations formally seek to prevent. The modelling process identified key system factors that enable high-risk industries to leverage the strengths of both reliability-based and resilience-based safety management approaches, while mitigating the limitations of each. Ultimately, the study expands the application of systems thinking and system dynamics by offering new theoretical and methodological insights, as well as practical tools for strategic organisational development in high-risk socio-technical settings.
