Available: https://scholar.sun.ac.za/items/76e88761-fbe0-4d80-901b-77c9c50a1e47 ENGLISH ABSTRACT: Background: Hypertensive disorders of pregnancy (HDP), particularly pre-eclampsia, are major contributors to maternal and perinatal morbidity and mortality. Despite extensive research, no single test reliably predicts preeclampsia. In resource-limited settings, a simple, affordable, and accessible tool is urgently needed. Maternal OAD has emerged as a potential screening and monitoring tool in hypertensive disorders of pregnancy. Objective: This study aims to assess and compare maternal ophthalmic artery Doppler velocimetric indices in pregnant women with hypertensive disorders (chronic hypertension and preeclampsia) in a South African population. Design: A prospective cohort study conducted at Tygerberg Academic Hospital, a tertiary referral centre in the Western Cape Province, South Africa. Methods: This prospective cohort study enrolled 134 pregnant women at ≥20 weeks’ gestational age at Tygerberg Academic Hospital. Women with chronic hypertension (n=67) and preeclampsia (n=67) underwent OAD assessment. The average peak ratio was calculated across six measurements (right and left eye, three waveforms each). Demographic variables (age, parity, BMI, HIV status) and antihypertensive treatment were analysed. Logistic regression models were used to evaluate factors associated with preeclampsia. Ethics: Ethical approval was granted by the Health Research Ethics Committee of Stellenbosch University. Written informed consent was obtained from all participants. Results: Women with preeclampsia were younger, more often nulliparous, and had significantly higher OAD peak ratios (median 0.79, IQR 0.68–0.89) compared to those with chronic hypertension (median 0.59, IQR 0.51–0.71; p<0.001). Peak ratios did not significantly differ between early- and late-onset preeclampsia. Conclusion: OAD peak ratios are significantly elevated in women with preeclampsia compared to those with chronic hypertension, supporting its role as a marker of cerebral vascular dysfunction. This study provides novel evidence in South Africa supporting OAD as a potential adjunct in HDP risk stratification, particularly in resource-limited settings.
