Available: https://scholar.sun.ac.za/items/32cc6c3b-2342-46bd-9c4c-de21a3e62cf4 ENGLISH SUMMARY: Background: The burden of stroke, one of the leading causes of death and chronic disability worldwide, is high in sub-Saharan Africa and continues to rise. Rehabilitation has the potential to reduce stroke-related disability and improve the quality of life of stroke survivors. Transitional Care Facilities offer a platform where in-patient stroke rehabilitation can be accessed within the public health sector of the Western Cape. Aim: This study aimed to describe the rehabilitation intervention received by stroke survivors admitted at a government-funded Transitional Care Facility as well as the resources available for this service provision. The experiences of the stroke survivors and rehabilitation staff were also explored. Method: A single-case case study design was utilised. Both qualitative and quantitative data were collected by means of a structured key informant interview, 7 semi-structured interviews with rehabilitation staff and stroke survivors, a document review of relevant policies, forms, templates and 12 patient folders, and direct observations of the facility. This data was analysed together for the purpose of triangulation. Descriptive statistics were used to analyse quantitative data. Qualitative content analysis was used to analyse rehabilitation session notes. Transcribed interview data was analysed by means of inductive thematic analysis using ATLAS.ti (v24). Findings: Stroke survivors were admitted at a median of 23.5(IQR 15.5-32.5) days post-stroke for 42 (IQR 37.75-51) days median length of stay. They received a median of 12 (IQR 8-17) rehabilitation sessions per admission across physiotherapy, occupational therapy and speech therapy. Essential equipment was available for rehabilitation, but availability of assistive technology was poor. Physical rehabilitation was the main focus of intervention with unmet cognitive rehabilitation, emotional support, assistive product and family training needs being identified. Two themes emerged regarding the experiences of these services by staff and stroke survivors, namely: “…nothing has changed to support that change – greater expectations for rehabilitation within an unchanged, unsupportive environment” and “But we keep on, we still make it work.” Conclusion: The Transitional Care Facility enabled access to rehabilitation during the important subacute period post-stroke. This was provided longer than what can be provided at acute hospitals and more frequent than what can be accessed at local clinics. The expectation for this service to be centred on a rehabilitation approach was unsupported by the staffing, resource allocation, physical environment and approach to nursing care.
