ENGLISH SUMMARY: Introduction: The occurrence of precautionary allergen labelling (PAL) on food products is estimated at 65%, but it is inconsistently applied. Consumers believe that PAL is regulated but their knowledge and practice of PAL is unsound, inconsistent and they have an apparent lack of trust in PAL statements. PAL should be clearly visible, easily understandable, reliable and precise in indicating either ‘real risk’ or ‘no risk’. Aims and objectives: The primary objective was to determine the knowledge and practice (including adherence) of South African parents/caregivers regarding precautionary allergen labelling (PAL) in the management of immune-mediated food allergies in children. Methodology: This study was a descriptive cross-sectional study with an analytical component. The study population consisted of parents/caregivers of children (birth to 10 years) with an immune-mediated food allergy in South Africa. An online self-administered questionnaire was the instrument for data collection. Results: Of the 140 participants included in the study, 98% were aware of a special statement on food labels indicating food allergens. A life-threatening reaction (p = 0.007) or a visit to the emergency unit (p = 0.033) was significantly associated with non-adherence to the PAL statement ‘made with the same equipment as (the allergen-containing food)’. The following phrases indicated the perceived real risk of allergen contamination: ‘contains’ (84%), ‘may contain’ (64%), ‘may contain traces’ (61%), ‘made with the same equipment’ (56%) and ‘made in the same factory’ (40%). Only 29% indicated that they would read the food label every time before they use a food product. Sixty-five percent of participants said they would give their child a food product with the label ‘made in the same factory’ and 25% would give their child a food product labelled ‘may contain traces’. Non-adherence to PAL is mostly a result of children not experiencing reactions after eating the allergen-containing food product. Conclusion: The participants’ understanding of PAL was inconsistent and few could confirm with confidence whether the allergen was present in the product. Most stated that the allergen ‘might be present’ and did not consider the product safe to consume, yet the risk of contamination was regarded as low. Many expressed the belief that the statement served mainly to protect the manufacturer. It is clear that parents of children with food allergies do not understand PAL or implement PAL statements effectively. It is vital, therefore, to standardise PAL globally to indicate ‘real risk’ or ‘no risk’ in order for consumers to recognise best practice in eliminating the risk of being exposed to allergen-containing food products. AFRIKAANSE OPSOMMING: Inleiding: Die aanduiding van voorkomende allergie etikettering (VAE) op voedsel produkte word geskat op 65% alhoewel die gebruik daarvan nie konstant is nie. Verbruikers glo dat VAE gereguleer word, maar hul kennis en toepassing van VAE is ongegrond, teenstrydig met geen vertroue in die verklarings op etikette nie. Dit is belangrik dat VAE duidelik sigbaar, maklik om te verstaan, betroubaar en presies moet wees, waar etikettering die ‘werklike risiko’ of ‘geen risiko’ aandui. Doelstelling en doelwitte: Die primere doelstelling was om die kennis en toepassing (insluitend nakoming) van Suid Afrikaanse ouers/voogde rakende VAE te bepaal in die bestuur van immuun bemiddelde voedsel allergiee in kinders. Metodes: Die studie was ’n beskrywende deursnit studie met ’n anatlitiese komponent. Die studie populasie was ouers/voogde van kinders (geboorte tot 10 jaar) met ’n immuun bemiddelde voedsel allergie in Suid-Afrika. Data-insameling was deur middel van ’n aanlyn self geadministreerde vraelys. Resultate: Van die 140 deelnemers was 98% bewus daarvan dat daar ’n spesiale verklaring op voedsel etikette is wat ’n voedsel allergeen aandui. ‘n Lewensgevaarlike episode (p = 0.007) of ‘n besoek aan die noodeenheid (p = 0.033) was volgens statistiek geassosieer met die versuim om die VAE ‘gemaak met dieselfde toerusting’ (as die allergeen)’ in ag te neem. Die ‘werklike risiko’ vir allergeen kontaminasie kon waargeneem word in die volgende frases: ‘bevat’ (contains) (84%), ‘mag bevat’ (may contain) (64%), ‘mag spore bevat’ (may contain traces) (61%), ‘gemaak met dieselfde toerusting’ (made with the same equipment) (56%) en ‘gemaak in dieselfde fabriek’ (made in the same factory) (40%). Slegs 29% het aangedui dat hulle die voedsel etiket elke keer voor die gebruik daarvan lees. Fyf-en-sestig persent van deelnemers sal ’n voedsel produk met ’n etiket ‘gemaak in dieselfde fabriek’ en 25% sal ’n voedsel produk met die etiket ‘mag spore bevat’ aan hulle kind gee. Die versuim om VAE in ag te neem is meestal as gevolg daarvan dat kinders geen reaksie toon nadat ’n voedselproduk geeet is wat die spesifieke allergeen aandui. Gevolgtrekking: Die deelnemers se begrip van VAE is teenstrydig en baie min kon met vertroue bevestig of ’n allergeen definitief in ’n produk teenwoordig is. Meeste sal noem dat die allergeen ‘miskien teenwoordig is’, die voedselproduk is nie veilig om te eet nie, alhoewel die risiko van kontaminasie min is. Baie glo dat die verklaring net die vervaardiger beskerm. Dit is duidelik dat ouers van kinders met ’n voedsel allergie nie VAE verstaan nie. VAE word ook nie effektief toegepas nie. Dit is noodsaaklik om VAE wereldwyd te standardiseer om ‘werklike risiko’ of ‘geen risiko’ aan te dui sodat die beste kennis toegepas kan word. Sodaande kan die risiko verminder word om voedsel in te neem wat blootgestel was aan allergeen kontaminasie.
