Malnutrition in children -- South Africa Dissertations -- Dietetics Thesis (MSc (Dietetics)) -- University of Stellenbosch, 1998. the study's main objectives were the identification of determinants of growth failure in 12 to 24 month old children visiting the Duncan Village Day Hospital and the identification of focus areas for intervention. this was accomplished by compiling a causal model, based on the UNICEF conceptual model for the development of malnutrition, to identify possible determinants. Subsequently a questionnaire was developed to assess the immediate and underlying causes of growth failure in the study population. Clinical and biochemical assessments were also included to evaluate current health and nutritional status. One hundred and fifty children were selected for the study using the convenience sampling method, with 100 children in the control group and 50 children in the growth failure group. The children int he growth failure group were matched with two children in the control group based on age and sex. Each child in the growth failure group was chosen according to anthropometric indices and had to have z-scores of < -2SD for weight for height and/or height for age, while the control group had to have z-scores of > -1SD for both weight for height and height for age. The control group children also had to have plasma albumin levels of ≥35g/dl. The data was gathered at the paediatric section of the Duncan Village Day Hospital over a one-year period. The control and growth failure groups were compared for all the variables so as to determine possible determinants of growth failure in the study population. The results indicate that, as far as immediate causes of growth failure are concerned, significant differences were found between the two groups in terms of disease and infection. The dietary intake of the total group seemed to be poor, with the only significant difference a lower frequency of milk intake by the growth failure group (Proc Mic: p=0.0344). However significant differences were found between the two groups for protein and anthropometric status, with growth failure children having had lower plasma albumin levels (RR* =12.2) and protein levels (RR=16). As disease as such was not significantly related to the growth failure problem, it is speculated that poor dietary intake was most probably a more important determinant of the low protein and anthopometrical status than is reflected by the dietary intake data. The latter needs to be interpreted with caution due to identified constraints in the dietary methodology.
